yourCARE+| Online education platform & videos
yourCARE+ is an online education platform accessible to patients and caregivers to assist with navigating home care services and managing chronic diseases, including heart failure and COPD. Some of my contributions to this project include:
Leading the adaptation, integration, and testing of the interRAI Self-Reported Check-Up assessment tool in yourCARE+.
Developing scripts and storyboards to create videos about the roles of home care service providers, caregiver stress and burnout, and community-based chronic disease management.
Conducting literature reviews and translating evidence into educational content to appear on the platform.
DIVERT COVID Toolkit | Online education platform
DIVERT COVID is a toolkit to support home care cardiorespiratory patients during the COVID-19 pandemic by disseminating knowledge (e.g., fact sheets) and providing resources. I collaborated with research staff to develop content for the educational materials.
